Fundamental Care

• Access to the CIGNA PPO Network**
• 51 or more eligible employees required
• Maximum coverage from $25,000-$100,000 per year
• Minimum of 50% employer contribution

** Fundamental Care is a directed-indemnity plan, not a PPO plan. However, individuals receive discounts when visiting CIGNA network health care professionals.
