Fundamental Care can be postitioned in three different ways:
As a core medical plan offered to employees who haven't had health insurance available through their employer.
As coverage for a carve-out population that desires richer benefits than limited-benefit plans provide
As coverage for employees who are in a waiting period and are not yet eligible for the employer's major medical plan.
Here are some important plan features:
Group Size: 51 or more eligible employees
Maximum coverage from $25,000 to $100,000 per year
Minimum of 25% employer contribution
