What is Starbridge Select?

Starbridge Select is a sickness and accident plan that is designed to help pay for common, everyday medical expenses. It is not a major medical plan or HMO. [top]

What type of medical expenses are covered?

Starbridge Select plans provide coverage for the treatment of sickness and off-the-job accidental injuries including:

Doctor visits
Lab and X-ray expenses
Hospital charges
Prescription drugs (depending on coverage level)
Maternity charges (depending on coverage level)

Do you have a list of doctors or hospitals that I must use?

No, because Starbridge Select is not an HMO or PPO you are allowed to see any licensed doctor of your choice. [top]

Are pre-existing medical conditions covered?

No, pre-existing medical conditions are not covered under Starbridge Select. A pre-existing condition is any condition for which you have been medically diagnosed, treated by or sought advice from, or consulted with a physician during the six months before becoming insured under the STARBRIDGE SELECT plan.  This provision will not apply to any expense incurred after the end of a continuous period of 6 months of coverage under the policy during which no expense is incurred, no diagnosis, treatment or advice is received and a physician has not been consulted; or 12 months of continuous coverage under the policy. (The above limitation does not apply to newborn or adoptive children, or any pregnancy. Definitions may vary from state to state. Please refer to your state's definition of a pre-existing condition.) If I leave my job, am I eligible for COBRA?

Yes, you have the option to keep your group health coverage, regardless of whether you are terminated or leave your position voluntarily. You will be notified within 60 days of your last day of employment of your option to enroll in COBRA. The coverages, by law, must be identical to the coverages provided to you while employed. You will be responsible for the full monthly premium amount. [top]
